Our staff is friendly and always ready to help with scheduling, insurance or anything else you may need during your time with us. For your convenience, we make the following information available:

Appointments:

To schedule an appointment with Gallas Plastic Surgery & Vein Center, please call our office at (281) 646-1114 or complete our online Request an Appointment form.

If at any time you are unable to keep your appointment, please notify us at least 24 to 48 business hours in advance. We will be glad to reschedule your appointment at a more favorable time. We greatly appreciate your time and consideration and look forward to seeing you.

New patient forms:

To help expedite your appointment with us, we have provided the following forms. You can either complete these on line prior to your appointment or you can print, fill them out and bring them with you. Online forms are securely stored in an encrypted database.
